Applications must be completed through your online portal and submitted with payment in full. Submitted applications are reviewed by staff.
If application deficiencies are identified, or the application must be presented for Board approval, the applicant will receive a notice of deficiency email prompting you to log into your online portal to make necessary corrections. Specific deficiencies are indicated within your application (click "Documents")
Standard Application Fee:
- PT: $260
- PTA: $160
Prorated Application Fee (applications submitted in year 2 of a renewal cycle; 9/1/odd year-8/31/even year):
- PT: $190
- PTA: $120

AZ testing eligibility requires a completed application submitted through your online portal.
Register and pay for exams through fsbpt.org (must be completed before eligibility can be granted).
ONLY after an application is paid, submitted, reviewed, and deemed sufficient (including 3rd party documents) will testing eligibility be granted.

Exams are administered at Prometric Testing Sites. NPTE is administered on fixed dates.

Congratulations, you've passed your exams and submitted your final transcripts (specifying graduation date, degree earned, and registrar signature). Now what?
Exam scores, once received will be added to the application file and the entire application will be reviewed again to ensure compliance.
Next, your application will navigate the final processing stages and once approved, your license or certificate number will be visible on the Online Registry.
Remember, final processing takes time! If issues arise at this stage, the applicant will be notified.
After your license/certificate is issued you will begin using your Licensee Portal for all other Board services and applications.

Fingerprint Clearance Card Information
A.R.S. §32-2022 requires that all initial applicants for licensure as a physical therapist and certification as a physical therapist assistant obtain and provide a valid Arizona Fingerprint Clearance Card issued by the Department of Public Safety as an application requirement. In addition, approval for testing on the National Physical Therapy Examination (NPTE) and the AZLAW examinations requires the Board of Physical Therapy have a copy of a valid fingerprint clearance card before approving applicants to test. Foreign Credentialing Commission on Physical Therapy (FCCPT)
Evaluator of foreign education credentials using Coursework Tool 5 (CWT5) which is required in AZ. Assists the U.S. Immigration and Naturalization Service (INS) and U.S. state licensing agencies by evaluating the credentials of Foreign Educated Physical Therapists (FEPTs) who wish to immigrate and work in the United States.

Foreign Educated Physical Therapist license applicants must have their foreign education credentialed using Coursework Tool 5 (CWT5).

International Consultants of Delaware (ICD)
Evaluator of foreign education credentials using Coursework Tool 5 (CWT5) which is required in AZ. ICD's mission is to provide educational evaluations of their client's educational credentials, in terms of their U.S. equivalence, without regard to sex, religion, country of birth or country of education.
International Education Research Foundation
Evaluator of foreign education credentials using Coursework Tool 5 (CWT5) which is required in AZ. The IERF Credentials Evaluation Service provides assistance in interpreting the educational backgrounds of people who have studied outside the U.S. and who need statements of U.S. educational equivalencies.
Request Score Transfer
When contacting the FSBPT for NPTE score transfers, individuals requesting transer must pay the FSBPT score transfer fee at the time of request.
