Renewal of a license/certificate is available only during the renewal window ending 8/31/even year. Renewal is applicable to all Active, Inactive/Not Practicing, and Inactive/Retired statuses. License/certificate renewal opens in early July of a renewal year and must be completed by August 31 of that same year. Failure to renew on time will result in an administratively suspended license/certificate.
NOTE:
- Licenses/certificates in Inactive/Not Practicing or Inactive/Retired status must be renewed on the same schedule as active licenses. Failure to renew on time will result in an administratively suspended license/certificate.
- If choosing to inactivate your license/certificate, the appropriate service request must be completed and processed BEFORE initiating a renewal application.
Fees:
- PT (Active) = $160
- PTA (Active) = $55
- PT (Inactive/Not Practicing) = $80
- PTA (Inactive/Not Practicing) = $27.50
- PT or PTA (Inactive/Retired) = $0
Steps to take:
- When the renewal portal is open, log into eLicense.az.gov and click Renew to submit an online renewal application
- Ensure both Mailing and Public addresses are selected within the renewal application (Board requires both address categories are supplied. Both categories may have the same address)
- Upload all supplemental documents in pdf format
- Answer Background questions pertaining to the compliance period preceding the renewal application (upload supporting documentation for any affirmative answer)
- Complete & upload Arizona Statement of Citizenship and Alien Status Form if indicated
- Upload front/back pdf copy of US Government Issued Identification if indicated
- Pay appropriate fees
- Ensure application indicates "Submitted" (Pending & Payment Pending indicates application is not yet submitted)
If reinstating an Inactive/Administrative Suspension license/certificate to active status. Reinstatement is available for license/certificates that have been in Inactive/Administrative Suspension status for less than 3 years from the most recent expiration date. Applicants eligible to reinstate will note a "Reinstate" link displayed within their eLicensing portal.
Fees:
- PT = $260 ($100 + $160 renewal fee)
- PTA = $105 ($50 + $55 renewal fee)
Steps to take:
- Log into eLicense.az.gov and click Reinstate to submit an online reinstatement application
- Upload all supplemental documents in pdf format
- Answer Background questions (upload supporting documentation for any affirmative answer)
- Complete & upload Arizona Statement of Citizenship and Alien Status Form
- Upload front/back pdf copy of US Government Issued Identification
- Complete and upload Reinstatement Forms Packet including continuing education completion documentation commensurate with the time of license/certificate lapse
- Pay appropriate fees
- Ensure application indicates "Submitted" (Pending & Payment Pending indicates application is not yet submitted)
If reactivating (reinstating) an Inactive/Not Practicing or Inactive/Retired license/certificate to active status.
Steps to take:
- Complete the appropriate form to reactivate from Inactive/Not Practicing or Inactive/Retired.
- Mail completed form, payment, and supporting documents to the Board offices.
A business entity that offers physical therapy services to the public and is not exempt from registration under A.R.S. §32-2030(H) must register each location from which PT services are offered in AZ.

A PT or PTA licensed/certified in another state but not licensed/certified in Arizona yet seeking to provide Interstate Telehealth Services into Arizona must submit an Interstate Telehealth Registration form pursuant to A.R.S. § 36-3606.
Fees = $100
- Complete the Interstate Telehealth Registration form.
- Mail completed form, payment, and supporting documents to the Board offices.
Interstate Telehealth registrations must be renewed/updated annually in accordance with the instructions. Complete this Patient Encounter Reporting Form and upload to your Interstate Telehealth registration renewal/update.
In accordance with A.R.S. § 32-2044(25), a physical therapist shall meet the qualifications established in A.A.C. R4-24-313(C) before providing the skilled intervention “dry needling”, as defined in A.R.S. § 32-2001(4).
- Complete the Dry Needling Training Submission form
- Email the completed form and supporting documentation to the email address provided on the form's letterhead. Supporting documents must clearly verify the information required by the form (if completion certificate does not clearly identify all required elements of training, additional supporting documents may be submitted)
- Anticipate an acknowledgement letter from ASBOPT when your documents have been reviewed and processed.

- If you permanently reside in AZ, compact privileges are not applicable. AZ residents, practicing in AZ, must be licensed through the AZ State Board of Physical Therapy. See New Applicants to submit an AZ PT License / PTA Certificate application
- AZ Compact privileges are purchased based on the eligibility of your home state license. AZ Compact privilege purchase requires successful completion of the Arizona Jurisprudence Exam (AZLAW) with EVERY purchase (original purchase and each subsequent purchase made in conjunction with your home state license renewal)
- Compact privilege is not an AZ license
- Compact privilege verifications may be obtained via ptcompact.org
- Register for the Arizona Jurisprudence Examination (AZLAW) through your fsbpt.org portal. NOTE: the AZLAW Jurisprudence Exam is administered at Prometric testing centers which has locations nationwide.
- Once registered send an email including your name, address and FSBPT ID to [email protected] requesting access to the Arizona jurisprudence exam for the purpose of obtaining a compact privilege.
