Renewal of a license/certificate is available only during the renewal window ending 8/31/even year. Renewal is applicable to all Active, Inactive - Not Practicing, and Inactive - Retired statuses. License/certificate renewal opens in early July of a renewal year and must be completed by August 31 of that same year. Failure to renew on time will result in an administratively suspended license/certificate.

NOTE:

  • Licenses/certificates in Inactive - Not Practicing or Inactive - Retired status must be renewed on the same schedule as active licenses. Failure to renew on time will result in an administratively suspended license/certificate.
  • If choosing to inactivate your license/certificate, the appropriate service request must be completed and processed BEFORE initiating a renewal application.

Fees:

  • PT (Active) = $160
  • PTA (Active) = $55
  • PT (Inactive - Not Practicing) = $80
  • PTA (Inactive - Not Practicing) = $27.50
  • PT or PTA (Inactive - Retired) = $0

Steps to take:

  1. When the renewal portal is open, log into your Licensee Portal and click Renew to submit an online renewal application
  2. Ensure that both a Mailing/Residential address and Employment Information are updated and accurate as a part of the renewal process. (Board requires both address categories are supplied.)
  3. Upload all supplemental documents in pdf format
  4. Answer all questions pertaining to the compliance period preceding the renewal application (upload supporting documentation for any affirmative answer)
  5. Complete & upload Arizona Statement of Citizenship and Alien Status Form if indicated
  6. Upload front/back pdf copy of US Government Issued Identification if indicated
  7. Pay appropriate fees
  8. Ensure application indicates "Submitted" 

If reinstating an Inactive - Administrative Suspension license/certificate to active status. Reinstatement is available for license/certificates that have been in Inactive - Administrative Suspension status for less than 3 years from the most recent expiration date. Applicants eligible to reinstate will note a "Reinstate" link displayed within their Licensee Portal.

Fees:

  • PT = $260 ($100 + $160 renewal fee)
  • PTA = $105 ($50 + $55 renewal fee)

Steps to take:

  1. Log into Licensee Portal and click Reinstate to submit an online reinstatement application
  2. Upload all supplemental documents in pdf format
  3. Answer all questions (upload supporting documentation for any affirmative answer)
  4. Complete & upload Arizona Statement of Citizenship and Alien Status Form
  5. Upload front/back pdf copy of US Government Issued Identification
  6. Complete and upload Reinstatement Forms Packet including continuing education completion documentation commensurate with the time of license/certificate lapse
  7. Pay appropriate fees
  8. Ensure application indicates "Submitted" 

If reactivating (reinstating) an Inactive - Not Practicing or Inactive - Retired license/certificate to active status, log into your Licensee Portal and use the left-hand menu to make the appropriate selection.

Fees depend on current Inactive status:

  • PT Inactive - Not Practicing to Active status = $80
  • PTA Inactive - Not Practicing to Active status = $27.50
  • PT Inactive - Retired to Active status = $160
  • PTA Inactive - Retired to Active status = $50

Steps to take:

  • In accordance with A.R.S. § 32-2031 or A.R.S. § 32-2032 (as applicable), an inactive licensee or certificate holder who applies to the Board for reactivation (reinstatement to active licensure or certification) within three years after the date the Board issues a notice of inactive status must submit the full annual renewal fee and prove to the Board’s satisfaction that the licensee or certificate holder has met continuing competence requirements as prescribed by the Board by rule A.A.C. R4-24-401 through R4-24-403
  • Complete all sections of the application, pay for, and submit the application for review. Please note that all application fees are non-refundable by law. 
  • Affirmative answer(s) to any Background question(s) require a narrative explanation and supporting documentation and may result in Board review. 
  • You must review and update any personal and business contact information, as well as employer information. 
  • All applicants are required to notify the Board immediately of any change of address, phone number, or name and complete those updates through their portal. 
  • When reactivating your license/certificate, you are required to upload proof of continuing competence. The total number of contact hours must equate to the lapsed compliance cycles. (Per cycle requirements PT = 20 contact hours; PTA = 10 contact hours) 
  • IF the Board is in an active renewal period, a licensee/certificate holder who is interested in reactivating from Inactive Not Practicing status must FIRST renew the Inactive license/certificate and THEN submit a Reactivation application.

  • CEU Completion Documents - To reactivate your license/certificate, you must provide proof of continuing competence equivalent to your period of inactivity. 
  • CEU Audit form - To reactivate your license/certificate, you must provide proof of continuing competence equivalent to your period of inactivity. 

A business entity that offers physical therapy services to the public and is not exempt from registration under A.R.S. §32-2030(H) must register each location from which PT services are offered in AZ.

A PT or PTA licensed/certified in another state but not licensed/certified in Arizona yet seeking to provide Interstate Telehealth Services into Arizona must submit an Interstate Telehealth Registration form pursuant to A.R.S. § 36-3606. Log into the Applicant Portal and select Interstate Telehealth Provider Registration.

Fees = $100

The Interstate Telehealth Provider application is for a PT or PTA licensed or certified in another state but not in Arizona yet seeking to provide Interstate Telehealth Services into Arizona must register as an Interstate Telehealth provider pursuant to A.R.S. § 36-3606. 

This registration is ONLY for purposes of providing interstate telehealth services to patients in Arizona. If you are seeking an Arizona PT license or PTA certificate submit the appropriate application from the New Applicant options. 

AZ certified PTs or certified PTAs treating AZ patients are NOT required to submit this registration form. 

INSTRUCTIONS 

Please read the following application instructions closely. All registration fees are non-refundable by law. 

The ability to engage in telehealth services for physical therapists residing outside of Arizona requires the completion of the Interstate Telehealth Provider Registration application to provide interstate telehealth service encounters with patients in the State of Arizona. A.R.S. § 36-3606 

A health care provider who is registered pursuant to this section may NOT: 

  • Open an office in this state except as part of a multi-state provider group that includes at least one health care provider who is licensed in this state through the Arizona State Board of Physical Therapy 
  • Provide in-person health care services to persons located in this state without first obtaining a license through the Arizona State Board of Physical Therapy 

You are requesting to register to provide telehealth services across state lines (into Arizona) which will require at a minimum that you: 

  • Hold a current, valid, and unrestricted license in another state and are NOT subject to any past or pending disciplinary proceedings in any jurisdiction where you hold a professional license. 
  • Provide verification proof of all professional licenses including all US jurisdictions in which the provider is licensed and the license numbers, including PT Compact privileges, and regardless of status. 
  • Provide evidence of and maintain Professional Liability Insurance Coverage. 
  • Provide proof of and maintain a Duly Appointed Statutory Agent for Service of Process in Arizona. 
  • Act in full compliance with all applicable laws and rules in this state. 
  • Consent to this state's jurisdiction for any disciplinary action or legal proceedings. 
  • Follow Arizona standards of care for the physical therapy profession. 
  • Notify the Arizona Board of Physical Therapy within 5 days after any restriction or disciplinary action is initiated or imposed on any license/certificate. 

If registering as a PTA, the supervising PT must also be an interstate telehealth registrant. PTAs must include in their registration the name, contact information, and the Arizona Interstate Telehealth Provider Registration (ITP) number of the supervising PT. 

An interstate telehealth provider DOES NOT need to complete an interstate telehealth registration if either of the following applies: 

  • The services are provided under one of the following circumstances: 
  1. a. In response to an emergency medication condition. 
  2. b. In consultation with a health care provider who is licensed in Arizona and who has the ultimate authority over the patient's diagnosis and treatment. 
  3. c. To provide after-care specifically related to a medical procedure that was delivered in person in another state. 
  4. d. To a person who is a resident of another state, and the telehealth provider is the primary care provider located in the person's state of residence. 
  • The health care provider provides fewer than ten telehealth encounters in a calendar year. 

DOCUMENTS TO BE UPLOADED BY THE APPLICANT

 

The following documents will be required to be uploaded to complete and submit in this application. You must upload a high quality image or PDF. 

Verification of licensure or certification - Upload Verification of professional/regulated licensure or certification from every jurisdiction in which you are currently or have been previously licensed/certified (not limited to physical therapy). Also include PT Compact privileges regardless of status. The verification must include:

  • Your Name
  • License number 
  • Date of issuance 
  • Current status 
  • Expiration date 
  • A statement of whether any disciplinary action is pending or has ever been taken against the applicant and if so, an explanation. 
  • NOTE: if this information is not available to you, it may require that you contact your licensing entity and request a license verification. 
  • A license verification is NOT a copy of your license.

Evidence of professional liability insurance coverage - Documentation must specifically state that the registrant name indicating the registrant is covered under the policy.

Proof of a duly appointed statutory agent on that agent's letterhead - Letter should prove that agent accepts the responsibility of acting as the statutory agent on the registrant's behalf.

Declarations - You will be asked to upload additional documentation relating to any "yes" responses in the Declaration section of this application

The following documents will be required only if applicable to you. You must upload a high quality image or PDF. 

Proof of Name Change - If your name on any supplied documentation differs from the name provided on your application, legal proof of name change must be uploaded. Legal proof of a name change must be a certified copy a divorce decree, marriage certificate, or other court ordered name change documentation.

RENEWAL/UPDATING OF AN INTERSTATE TELEHEALTH REGISTRATION

Interstate Telehealth registrations must be renewed/updated annually through the Licensee Portal. Complete this Patient Encounter Reporting Form and upload to your Interstate Telehealth registration renewal/update.

 

In accordance with A.R.S. § 32-2044(25), a physical therapist shall meet the qualifications established in A.A.C. R4-24-313(C) before providing the skilled intervention “dry needling”, as defined in A.R.S. § 32-2001(4)

  1. Complete the Dry Needling Training Submission form
  2. Email the completed form and supporting documentation to the email address provided on the form's letterhead. Supporting documents must clearly verify the information required by the form (if completion certificate does not clearly identify all required elements of training, additional supporting documents may be submitted)
  3. Anticipate an acknowledgement letter from ASBOPT when your documents have been reviewed and processed.

  • AZ Compact privileges are purchased based on the eligibility of your home state license. AZ Compact privilege purchase requires successful completion of the Arizona Jurisprudence Exam (AZLAW) with EVERY purchase (original purchase and each subsequent purchase made in conjunction with your home state license renewal)
  • Compact privilege is not an AZ license
  • Compact privilege verifications may be obtained via ptcompact.org
  • Register for the Arizona Jurisprudence Examination (AZLAW) through your fsbpt.org portal. NOTE: the AZLAW Jurisprudence Exam is administered at Prometric testing centers which has locations nationwide.
    • Once registered send an email including your name, address and FSBPT ID to [email protected] requesting access to the Arizona jurisprudence exam for the purpose of obtaining a compact privilege.
